You can download the Veterinary Consent form here. Unfortunately, I cannot treat your dog or horse without it in guidance of the Veterinary Surgeons Act 1966 and Veterinary Exemptions order, 1962. Your vet will ensure that your dog or horse is fit for treatment and to disclose any relevant medical history to ensure we can provide the best possible care. If you have any questions regarding your pets suitability for treatment, please do contact your Veterinary Surgeon or myself directly.

I am more than happy to see patients further afield, but any travel over 20mile will incur a 50p per mile charge which will be discussed with you upon booking .
